PD TeleCare: The Supportive Teammate Transforming Patient Outcomes

Before we implemented the PD TeleCare trial, our patient loss rate was approaching 20% within the first six months. Our clinical team sometimes took over two days to respond to alerts from patients managing their care at home.

But since June 2021, things have dramatically improved. With Vantive’s PD TeleCare support service, the centers' response time to patient alerts has decreased from over two days to less than one, ensuring timely outreach and support.
